If your child needs medication during the school day, a medication in school form must be completed and signed by both the health care provider and parent/guardian and submitted to the school.

Students with life-threatening health conditions, including diabetes, asthma, seizure disorders and severe allergies, must also have the necessary medication and medical supplies available at school.

Please bring all current medical documentation, completed medication forms, medications and required supplies to the school as soon as possible. The school nurse will use this information to develop or update your child’s health plan and help ensure staff are prepared to safely support your child during the school day.
